At Home Solutions, LLC provides Home Management and Caregiver support services. We serve older adults and adults with disabilities who need assistance to remain independent wherever they are living. Our client’s comfort, safety and well-being are our top priorities.

We have two programs to choose from:

Home Management Program

§ A home safety assessment to reduce the risk of falls and ensure longevity at home.

§ Simple home repairs, improvements and modifications to live independently and safely – like grab bars, installing raised toilet seats, temporary or permanent ramps.

§ Pre-screened, trustworthy service companies who can assist with simple to major modifications.

Daily Living Support Program

Caregivers provide:

o companionship, socialization

o incidental transportation

o laundry, linen changing

o light homemaking

o meal preparation

o downsizing/organizing and decluttering

o medication reminders

o stand by showering

o hospice supportive care

o respite support service

o Alzheimer’s and dementia care

o Parkinson’s care

o House/pet sitting

o Gardening / plant care

§ Our Caregivers are employed by us (not contractors) and are thoroughly interviewed, screened, trained, supervised and are required to attend regular company educational meetings.

§ We strongly emphasize open and honest communication with our client’s primary contact on a consistent basis.

§ We also provide compassionate respite services for family members who need a break from their primary Caregiver needs.

The Eden Prairie Senior Center is located at 8950 Eden Prairie Road. The Center is open Monday-Friday from 8:30 am to 4:00 p.m. The Senior Center mission is to provide for social, recreational, educational, psychological and physical needs; to enhance the visibility of older adults; and to promote participation in all aspects of community life. Below is a list of the growing number of social and recreational programs for all adults; Weekly events: shopping bus, quilting group, duplicate and partner bridge, walking club, 500 cards, game day, Wii bowling, polka band, cribbage, men's social group, linedance class. Monthly events: movie matinee, welcome coffee, educational classes, bunco, book club and a variety of special events such as summer BBQ, fall festival meal, holiday lunch, garden party ice cream social, Valentines meal, Golf Classic. Free Health Services: health and blood pressure clinic, insurance help for seniors. Other fee for service health services: foot care clinic $31. We also support a independant Red Hat group. See our quarterly Senior Center News letter for more special events and upcoming trips and tours.   Transportation is available to Senior Center programs. The Senior Center building is availble for rental for speical events or meetings. The Eden Prairie Library is a great place to go for programs, classes, Internet access, or to find your next great read.

Opened in 2004, the 40,000 square foot library houses a collection of 150,000 items, 82 public computer workstations, a comfy fireplace and reading lounge, space for quiet and group study, and reservable meeting space—all available at no charge.

Hennepin County Library’s mission is to nourish minds, transform lives, and build community together. The 41-library system serves 1.1 million Hennepin County residents and offers more than 5 million books, CDs, DVDs and other items in more than 40 languages and has 1,750 public computers.

Focus areas include early literacy, out-of-school time, jobs and small business support, lifelong learning, new immigrants and individuals 55+. The Library also serves County residents through an extensive website (www.hclib.org) and outreach to customers who are homebound, in senior residences, or in correctional facilities.

Prairie Adult Care provides licensed health care and recreation during day hours in the protective environment of a day center. Participants attend the day center as needed Monday through Friday for socialization, medical and nutritional monitoring, and rehabilitation. Transportation is provided to and from your home. SarahCare a premier adult day care facility, is a participant-driven program developed in 1985 by internationally renowned gerontologist Dr. Merle Griff, Ph.D. in Canton, Ohio. The unique program begins with the individual and reaches outward to involve the community as a whole. SarahCare offers unparalleled assistance to the participant's special needs, but more importantly recognizes the uniqueness of each individual. Beyond our professional staff of nurses and specialists helping with daily tasks of living, SarahCare seeks to share in the richness of the individual's personal life and actually customize the environment to stimulate the client's senses.

Our program is designed to encourage the vulnerable adult to remain involved in activities of their choice.
