At Home Solutions, LLC provides Home Management and Caregiver support services. We serve older adults and adults with disabilities who need assistance to remain independent wherever they are living. Our client’s comfort, safety and well-being are our top priorities.

We have two programs to choose from. Our Home Management Program provides a home safety assessment. This comprehensive safety evaluation focuses primarily on the structural integrity of the home along with assessing the physical environment to reduce the risk of falls. We also provide pre-screened service companies who can assist with any repairs, improvements and modifications needed to live more independently and safely.

The Daily Living support services our Caregivers provide include: companionship, transportation, running errands, laundry, light homemaking, meal preparation, downsizing/organizing and decluttering; medication reminders and stand by showering. Our Caregivers are thoroughly interviewed, screened, trained and are required to attend on-going company educational meetings. We strongly emphasize open and honest communication with our client’s primary contact on a consistent basis. We also provide compassionate respite services for family members who need a break from their primary Caregiver needs.

Prairie Adult Care provides licensed health care and recreation during day hours in the protective environment of a day center. Participants attend the day center as needed Monday through Friday for socialization, medical and nutritional monitoring, and rehabilitation. Transportation is provided to and from your home. “Senior Outreach” is part of Senior Community Services and provides in-home counseling and care management to elders and their families. Licensed staff assist seniors to identify and connect with the right combination of services to allow them to remain in their homes. The H.O.M.E. Program (Homemaking and Outside Maintenance for the Elderly) can be set up by the “Senior Outreach” social worker. Costs are on a sliding fee and services include cleaning, laundry, shopping, snow removal, lawn care, interior/exterior painting, minor repairs, etc. Senior Community Services also has a “Health Insurance” program to answer questions on Medicare, supplemental coverage, long-term care insurance, bill sorting and appeals.
