Prairie Adult Care provides licensed health care and recreation during day hours in the protective environment of a day center. Participants attend the day center as needed Monday through Friday for socialization, medical and nutritional monitoring, and rehabilitation. Transportation is provided to and from your home. PROP is an all-volunteer, non-profit community human service organization that serves residents of Eden Prairie and Chanhassen who are in need of emergency help. “Senior Outreach” is part of Senior Community Services and provides in-home counseling and care management to elders and their families. Licensed staff assist seniors to identify and connect with the right combination of services to allow them to remain in their homes. The H.O.M.E. Program (Homemaking and Outside Maintenance for the Elderly) can be set up by the “Senior Outreach” social worker. Costs are on a sliding fee and services include cleaning, laundry, shopping, snow removal, lawn care, interior/exterior painting, minor repairs, etc. Senior Community Services also has a “Health Insurance” program to answer questions on Medicare, supplemental coverage, long-term care insurance, bill sorting and appeals.
